A Brief Overview

The Nurse Coordinator role encompasses a range of specialized nursing positions dedicated to providing professional support and coordination for patient care across various healthcare settings. They ensure compliance with provider orders and established policies while utilizing the nursing process to optimize patient outcomes. Nurse Coordinators play a crucial role in assisting and supporting patients in accessing and managing their healthcare needs, collaborating with the care team to facilitate seamless coordination of patient care services. Responsibilities include patient communication, education, preparation for procedures, identifying opportunities for improvement, and supporting clinic management in addressing operational needs. As an integral member of the care team, Nurse Coordinators contribute significantly to enhancing patient experiences and overall healthcare efficiency.



What You Will Do

  • Delivers age-specific, patient-centered care with sensitivity and respect for the diversity of the human experience, following the nursing process and standards of practice.
  • Demonstrates critical reasoning and astute clinical judgment to expedite appropriate care and treatment, particularly when dealing with complex problems or potentially life-threatening conditions.
  • Acts as a partner, advocate, and advisor, assisting patients and their families in managing their healthcare effectively, respecting their unique values, needs, health goals, and treatment preferences.
  • Conducts assessments, triages patients, and provides nursing consultation, follow-up, and care coordination during face-to-face encounters or via telecommunications technologies.
  • Demonstrates leadership by motivating and acknowledging staff contributions, promoting team building, and participating in staff competency development and evaluation.
  • Prioritizes productivity, completing reports and projects in a timely manner, setting clear goals, and managing time effectively.
  • Implements relevant nursing interventions, including identifying and addressing patient needs, offering health education, advocating for patients, coordinating healthcare services, assisting in navigating the healthcare system, and evaluating patient outcomes.
  • Collaborates with inpatient and clinic staff to develop patient-appropriate informational materials and educational content. Assesses learning needs and designs curriculum accordingly.
  • Engages patients through face-to-face encounters and various telecommunication strategies, fostering long-term relationships and creating a supportive environment.
  • Ensures continuity of care by utilizing the nursing process, collaborating with interprofessional teams, and providing access to appropriate healthcare services and community resources across the care continuum.
  • Embraces lifelong learning, continuously updating and expanding clinical, organizational, and professional knowledge and skills in the field.
  • Utilizes performance improvement methodology to design data collection systems/processes and identify opportunities for enhancing patient care and service.
  • Prioritizes patient safety and consistently upholds high-quality nursing care during every encounter.
  • Performs other duties as assigned, including On Call responsibilities.

Education Qualifications

  • Bachelor’s degree or equivalent from an accredited college/university Required

Experience Qualifications

  • Two (2) years of progressively responsible and directly related work experience

  • Knowledge as follows:
    • in specialty area of practice
    • of current theories, principles, practices, standards, emerging technologies, techniques and approaches in the nursing profession, the health care system, the responsibility and accountability for the outcome of practice
    • of computer systems and software used in assigned work area
    • of laws, rules and regulations; standards and guidelines of certifying and accrediting bodies; department standards, protocols, policies and procedures governing the provision of nursing care in the assigned work area
    • and skills in teaching, educating, problem-solving, and critical thinking
    • of medical and professional nursing ethics and patient privacy rights
    • of medical terminology; principles and practices of health promotion, risk reduction, illness and disease prevention and management; medications and drugs, common dosages, their physical and physiological effects, and possible adverse reactions

Licenses and Certifications

  • California Registered Nurse (RN) license
  • Basic Life Support (BLS) certification for providers issued by the American Heart Association
